Temporary Care Management & Transportation Assistant
Pay: $35.00 per hour + mileage reimbursement

Schedule: Flexible; weekday, evening, and occasional weekend availability may be needed
About the Position
A Care Management & Transportation LLC is seeking a dependable, compassionate, and professional Temporary Care Management & Transportation Assistant to support clients with care coordination, appointments, errands, transportation, and day-to-day non-medical needs.
This position is ideal for someone who enjoys working with older adults, individuals with disabilities, and families who need extra assistance navigating appointments, transportation, and community services.
Responsibilities
• Provide safe and reliable transportation to medical appointments, therapy, shopping, social activities, and other scheduled destinations
• Accompany clients to appointments when requested
• Assist clients with checking in, scheduling follow-up appointments, and organizing appointment information
• Communicate appropriate updates to the care manager or designated family representative
• Assist with errands, prescription pickup, grocery shopping, and other authorized client needs
• Help coordinate appointments and transportation schedules
• Maintain accurate mileage, service, and client activity records
• Provide courteous door-to-door assistance when appropriate
• Maintain client privacy, professionalism, and confidentiality
• Observe and report changes or concerns to the care manager
• Follow company policies and client care plans
• Perform other non-medical care-management support duties as assigned
This is a non-medical position. Employees do not provide nursing care, administer medications, or perform medical procedures.
Requirements
• Valid California driver’s license
• Clean and acceptable driving record
• Reliable transportation
• Current automobile insurance
• Ability to pass a background check
• Dependable, punctual, and professional
• Excellent communication and interpersonal skills
• Comfortable working with seniors, individuals with disabilities, and their families
• Ability to use a smartphone for navigation, scheduling, text communication, and documentation
• Ability to maintain client confidentiality
• Flexible availability
• Legally authorized to work in the United States
Preferred Qualifications
• Previous experience in care management, caregiving, healthcare, social services, transportation, senior services, or patient coordination
• CNA, Medical Assistant, LVN/RN, social-services, or healthcare background is a plus but not required
• CPR/First Aid certification preferred
• Experience transporting or assisting older adults or people with mobility limitations
• Familiarity with the San Fernando Valley and Greater Los Angeles area
Compensation
$35.00 per hour plus mileage reimbursement for authorized business mileage.
Hours will vary depending on client needs. This is a temporary/on-call position with the possibility of additional assignments as the company grows.
