Nurse Practitioner (NP) - Geriatric Care - Skilled Nursing Facilities / Nursing Homes

Location: Sarasota, FL Area

Overview:

We are seeking compassionate and motivated Nurse Practitioners (NPs) with a strong interest in geriatric care to join a physician-led organization serving Skilled Nursing Facilities (SNFs) and nursing homes. The focus of this role is on providing high-quality patient care, supporting positive outcomes, and working to reduce avoidable hospitalizations.

This is an excellent opportunity for both experienced NPs and new graduates looking to develop their skills in post-acute and long-term care medicine.

About the Role:

Nurse Practitioners in this position will provide direct patient care in a territory of Skilled Nursing Facilities and nursing homes. Over time, you will work closely with supervising physicians and collaborate with other advanced practice providers to ensure continuity and quality of care.

Responsibilities:

Provide comprehensive primary and acute care to geriatric patients in Skilled Nursing Facilities and nursing homes.
Manage chronic conditions, monitor acute changes, and help reduce hospital readmissions.
Perform daily rounds and evaluate patients' medical needs in collaboration with facility staff.
Coordinate care with physicians, therapists, nurses, case managers, and families.
Ensure documentation accuracy and compliance with facility and regulatory requirements.
Promote patient safety, wellness, and quality of life through proactive care planning.

Qualifications:

Licensed Nurse Practitioner (NP) in Florida, in good standing.
Experience in geriatrics, long-term care, or Skilled Nursing Facilities preferred.
New graduates are welcome to apply with demonstrated interest in geriatric medicine.
Strong clinical assessment, decision-making, and communication skills.
Ability to work independently while collaborating with an interdisciplinary team.
