Key Responsibilities: As a Home Health RN, you will:
- Conduct Home Health Nursing Assessments to determine equipment needs, participant preferences, and care goals.
- Deliver and document home health nursing interventions in alignment with care plans, including medication administration, physician orders, and maintaining a safe environment.
- Educate participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies.
- Track and monitor Home Health scheduling in collaboration with the Home Health Manager or scheduling point of contact.
- Work closely with Physicians and the Interdisciplinary Team (IDT) to ensure smooth care transitions between hospitals, skilled nursing facilities, and home settings.
Basic Qualifications: - Graduate of an ccredited School of Nursing.
- Unencumbered RN License required.
- Minimum one year of nursing experience in a clinical setting, preferably with a frail or elderly population. Home health agency experience is a plus.
- Reliable transportation for home visits.
