Job Summary
We are seeking a compassionate and dedicated Endocrinology Nurse Practitioner (NP) to join our dynamic team. In this role, you’ll play a vital part in delivering comprehensive endocrine care to patients, working closely with an interdisciplinary team in both long-term care and acute settings.

Responsibilities
• Provide on-site endocrinology consults in nursing home settings, including: o Initial assessments for new admissions and complex endocrine conditions (e.g., diabetes, thyroid disorders, osteoporosis, adrenal disorders) o Routine weekly follow-up visits and care adjustments based on findings
• Conduct comprehensive evaluations: o Gather medical histories and perform endocrine-focused physical exams o Order and interpret labs including A1C, thyroid panels, metabolic panels, bone density tests, and other diagnostics
• Collaborate with endocrinologists, interdisciplinary teams, nursing staff, and facility providers to develop treatment plans, manage complex metabolic conditions, and reduce hospital readmissions
• Administer and manage medications including insulin and endocrine therapies, monitor treatment responses, and adjust regimens appropriately
• Educate patients, residents, and families on diabetes management, nutrition, hormone disorders, and medication adherence
• Provide emotional support and counseling to patients and caregivers
• Ensure accurate documentation in compliance with OSHA and governmental regulations, including thorough documentation in EMRs and transition summaries to ensure continuity of care across settings
• Maintain familiarity with CMS regulations and NCQA HEDIS guidelines
• Participate in quality improvement programs aimed at improving chronic disease management in SNFs
Qualifications
• Minimum of 6 months of experience in a long-term care or skilled nursing facility (Hospital/outpatient office experience will also be considered)
• Active and unrestricted NP license • DEA license required • CPR certification (BCLS/BLS) • Master’s degree in Nursing (NP applicants)
• Graduation from an accredited NP Program
• Preferred: Experience in endocrinology (training available if not)
• Preferred: Familiarity with EMR systems (e.g., Sigma, Visual, PCC – training provided)
• Strong organizational, communication, and time management skills
