The Registered Nurse (RN) provides nursing assessment, monitoring, and protocol-driven intervention for adults
experiencing acute intoxication within a low-barrier sobering environment. The RN performs abbreviated nursing
triage per standardized procedure, which assesses behavioral and physical health needs, and documents in
accordance with harm-reduction and trauma-informed best practices. This role ensures safe oversight, supportive
care, and protocol-guided referrals or transfers for individuals who may otherwise be transported to a hospital or jail.
Perform abbreviated nursing assessments, including vitals, impairment/intoxication levels, and
withdrawal risk screening.
• Recognize signs of medical or psychiatric emergencies and initiate EMS or ED transfers when clinically
indicated.
• Provide supportive care, including hydration, nutrition/snack support, wound care, and medication
administration per standing orders for minor injuries or symptoms.
• Assess and monitor patients through the sobering process, noting changes in behavior, cognition, or level
of consciousness.
• Offer education, motivational engagement, and compassionate communication regarding substance use,
health risks, and harm-reduction strategies.
• Utilize trauma-informed and de-escalation approaches to respond to escalating behaviors.
• Conduct incident debriefs and complete required clinical documentation following escalations or
emergency outcomes.
• Collaborate, as appropriate, with EMS, law enforcement, and emergency department teams as part of
alternate destination protocols.
• Support center-wide safety and risk mitigation practices across the multidisciplinary team.
• Participate in required onboarding and ongoing clinical training; maintain competencies and attend
required meetings.
• Perform all other duties as assigned within the scope of practice.
