Position Summary Clinical Chart Abstractor (NLP Validation) is responsible for reviewing medical records to abstract key clinical data and validate NLP-generated outputs against source documentation. This role ensures data accuracy, completeness, and clinical consistency to support analytics, research, quality measurement, and product development while maintaining strict adherence to privacy, security, and documentation standards.
Key Responsibilities - Abstract specified clinical data elements from electronic health records (EHRs), including unstructured notes (e.g., H&P, progress notes, discharge summaries, operative reports) and structured fields (e.g., labs, meds, diagnoses, procedures).
- Validate NLP outputs by comparing extracted entities, attributes, and context to source documentation.
- Identify and label discrepancies (false positives/negatives, boundary errors, concept mapping issues) using defined annotation/validation guidelines.
- Apply clinical and coding knowledge to normalize concepts (e.g., SNOMED CT/ICD-10-CM/CPT/RxNorm/LOINC) when required by the project.
- Document validation decisions clearly, including citations to note section/date/time and rationale aligned to guidance.
- Support inter-rater reliability (IRR) activities: participate in calibration sessions, double-review workflows, and adjudication discussions.
- Track production metrics (volume, turnaround time, error types) and escalate ambiguous cases or potential guideline gaps to leads.
- Provide feedback to NLP/ML, data science, or product teams on recurring failure modes and opportunities to improve rules/models/training data.
- Maintain strict patient privacy and data security practices in accordance with HIPAA and organizational policies.
Minimum RequirementDegree or equivalent and typically requires 4+ years of relevant experience.
Critical Skills- 3+ years of experience in clinical chart abstraction, clinical data review, medical record auditing, clinical documentation improvement (CDI), HEDIS/quality abstraction, or related work.
- Demonstrated ability to interpret clinical documentation, including common abbreviations, problem lists, medications, labs, and imaging/diagnostic results.
- Familiarity with EHR navigation and chart review workflows.
- Strong attention to detail with consistent application of written guidelines and high accuracy in repetitive review tasks.
- Basic understanding of NLP concepts (e.g., entity extraction, negation, context) or willingness to learn quickly.
- Proficiency with common productivity tools (e.g., Excel) and web-based annotation/validation platforms.
- Ability to handle sensitive data with discretion and comply with privacy/security requirements.
Preferred Qualifications - Clinical credential or background (e.g., RN), preferably in Oncology.
- Experience with oncology abstraction, or research data collection.
- Experience working with clinical terminologies and code sets (ICD-10-CM, CPT/HCPCS, SNOMED CT).
- Prior experience with annotation tools (e.g., LabelStudio) or similar validation workflows.
- Exposure to data quality processes, IRR studies, or gold-standard dataset development.
Additional Skills- Ability to synthesize information across multiple encounters and note types to determine the most accurate clinical truth.
- Understanding of clinical context elements relevant to NLP validation (negation, historical vs current, family history, ruled-out conditions, suspected/possible diagnoses, medication start/stop).
- Clear, concise written communication for documenting decisions and escalating issues.
- Time management skills to meet productivity and turnaround expectations without sacrificing quality.
- Comfort working in ambiguity and contributing to continuous improvement of guidelines and processes.
