Heart of Humanity Home Health Care is seeking a compassionate and dependable LVN to provide one-on-one nursing support for an individual receiving licensed services through Heart of Humanity Home Health Care. Monday through Friday 9 am to 6 pm.
The client requires skilled nursing oversight related to respiratory care, nutritional support, medication administration, safety monitoring, and activities of daily living. The LVN will work closely with the client, family, supervising nurse, and care team to follow the established plan of care.
Primary Responsibilities
• Monitor the client’s condition and complete ongoing nursing assessments
• Administer medications and treatments as ordered
• Provide respiratory treatments and monitor respiratory status
• Perform aspiration precautions and monitor for signs of respiratory distress
• Provide feeding support through a J-tube and follow prescribed nutritional instructions
• Monitor skin integrity and provide preventative skin care
• Assist with repositioning, mobility, transfers, and personal care needs
• Maintain infection-control and universal precaution standards
• Document services, observations, treatments, and any changes in condition
• Follow the client’s emergency and safety plan
• Notify the supervising nurse and physician of significant changes in condition
• Provide education and support to the client and family within the LVN scope of practice
Qualifications
• Current and unrestricted California LVN license
• Current CPR/BLS certification
• At least one year of nursing experience preferred
• Experience with J-tubes, respiratory treatments, aspiration precautions, and medically complex clients strongly preferred
• Ability to safely perform transfers and assist with mobility
• Strong documentation and communication skills
• Reliable transportation and consistent attendance
• Ability to pass required background and health clearances
