Norris Family Home Care is seeking an experienced, compassionate, and highly organized Care Coordinator / Field Supervisor to join our growing team. This is a primarily remote position that combines client care management with field responsibilities throughout San Diego County.
The ideal candidate enjoys working independently, building relationships with clients and families, and ensuring every client receives exceptional care. This is a leadership position requiring both administrative and hands-on caregiving experience.
Position Summary
As our Care Coordinator / Field Supervisor, you will serve as the primary field representative for Norris Family Home Care. You will conduct in-home assessments, perform supervisory and customer satisfaction visits, support caregivers, and step in as a backup caregiver when staffing needs arise.
This position is perfect for someone who enjoys variety—no two days are the same.
Responsibilities
Conduct comprehensive in-home client assessments and reassessments
Develop and update client care plans
Complete supervisory and quality assurance visits
Perform routine customer satisfaction visits with clients and families
Maintain strong relationships with clients, families, hospitals, hospice agencies, rehabilitation centers, and referral partners
Support and coach caregivers in the field
Assist with caregiver training and orientation
Fill in as a backup caregiver during caregiver call-outs or emergencies
Document client visits accurately and promptly
Assist with scheduling and care coordination when needed
Ensure compliance with California Home Care Services Bureau regulations and Norris Family Home Care policies
Qualifications
Minimum 3 years of professional caregiving experience (5+ years preferred)
Previous experience conducting home care assessments, supervisory visits, or client care coordination
CNA certification preferred
Experience working in home care, assisted living, skilled nursing, or rehabilitation settings
Excellent customer service and communication skills
Strong organizational and problem-solving abilities
Comfortable working independently while managing multiple priorities
Valid California driver's license
Reliable vehicle with proof of insurance
Must be willing to travel throughout San Diego County
Ability to pass all required background screenings and meet California Home Care Services Bureau requirements
CPR/First Aid certification preferred
