Overview*All positions are located in Fresno/Clovis CA*
ResponsibilitiesProvides for delivery of basic pharmacy services in the Ambulatory Care setting. Reviews all drug orders and checks all work completed by ancillary personnel in the department. Provides medication consultation and drug information at point of sale. Serves as drug expert and consultant to physicians, nursing staff and other hospital personnel. Performs drug utilization review on all prescriptions including monitoring for inappropriate controlled substances usage. Accountabilities 1. Performs drug utilization review on all dispensed medications reviewing patient profile for allergies, interactions and age appropriateness.2. Intervenes with provider when appropriate for clarification and correction of medication orders. Pursues opportunities for insurance formulary adherence to enhance patient compliance.3. Reviews and verifies all drug order entry, packaging and labeling by Pharmacy Technicians to assure accuracy and appropriateness for all dispensed medications.4. Directs pharmacy support personnel in the prescription dispensing process to ensure accuracy and timeliness.5. Prepares extemporaneous compounded medications as described in the non-sterile compounding policy and procedures.6. Maintains controlled substance records and documentation in accordance with local, state and regulatory law.7. Analyzes prescribing trends for narcotics and controlled drugs with CURES website to monitor patient compliance and to prevent excessive usage or harmful interactions.8. Provides medication consultation and drug information at point of sale. Addresses conflicts with customers as necessary.9. Serves as a drug expert and consultant to physicians, nursing staff and other personnel.10. Orders controlled substance medications from wholesaler and non-controlled medications when buyer is not available.11. Be involved in a specialty area of the pharmacy as assigned and provide that expertise to other staff.12. Takes part in the quality assurance program to improve workflow and reduce medication errors.13. Completes assigned individual projects to improve overall performance of the pharmacy.14. Assists in the training and mentoring of pharmacist and pharmacy technician students.15. Performs other duties as assigned.
QualificationsEducation• Bachelor's Degree in Pharmacy required• Doctor of Pharmacy (PharmD) preferred Experience• 2 years of Ambulatory Care Pharmacy experience preferred Licenses and Certifications• RPH - Registered Pharmacist with valid State of California Pharmacist License required
