Transforming Maternal Health (TMaH) Quality Improvement Specialist, who will be part of South Carolina's Transforming Maternal Health Model (TMaH) Team funded through a cooperative award with Center for Medicare and Medicaid services which will test whether effective implementation of a package of evidence-informed interventions, sustained by a value based payment model can improve maternal outcomes in a fiscally responsible way.
The Quality Improvement Specialist (QIS) will play a pivotal role in leading a transformative maternal health care improvement project aimed at fostering team-based care across multiple disciplines to improve maternal health outcomes. This position will work to enhance care delivery models through collaborative strategies that bring together various service delivery sectors, enabling the delivery of whole-person care. The QIS will guide the implementation of evidence-based care models such as AIM bundles, utilizing learning collaboratives to foster teamwork and shared learning across providers.
The ideal candidate will have expertise in quality improvement (QI), value-based payment models, and maternal health care. The QIS will be responsible for developing, designing, and executing QI projects from initiation to completion, leveraging data-driven decision-making, and applying proven strategies to deliver measurable improvements in maternal care outcomes.
- Develop plans to present information on all practice data and the development of an individual plan to work with each engaged practice to include: an analysis of QI project by overall conditions. Work with each engaged practice to identify trends, implement successful cycles into office routine, and plan for sustainability of selected quality improvement processes. Work with TMaH participants to implement AIM safety bundles across disciplines.
- Works with CMS, TMaH staff and contractors and practices to identify TMaH areas of focus and data capture. Prepare reports and advise management on findings related to the data submitted by participating practices. Provide recommendations based on benchmarking with state and national data. Work with staff and practices in the identification, collection, and documentation of data on TMaH related topics of interest and their related activities. Collect and perform analysis on quality improvement and other intervention documentation/data. Analyze Medicaid administrative claims, practice specific QI data, CMS Core Maternal Measures and HEDIS data to assess the impact of TMaH initiatives.
- Direct the creation and delivery of quality improvement skill development programs for the TMAH team within DHHS and the TMAH practices. Assist and coach practice staff at each TMAH practice with their understanding of quality improvement (QI) techniques. Provision of QI training - both at practice and system level. Host QI workshops, calls and QI coordinator groups.
- Communication and guidance provided to the practices and stakeholders. Provide information to the project director as evidenced by the planning and organization of work activities. Present information at various meetings (site visits, Learning Collaborative sessions and workshops). Establish effective working relationships. Actively participate in the Learning Collaborative and all TMaH activities. Learning Collaborative can be virtual, in-person, off-site, and may require overnight or weekend travel.

Minimum and Additional Requirements A bachelor's degree and at least five (5) years of professional experience related to quality improvement in a health care or social service setting.
Preferred: Master's degree in nursing, public health, health care administration or related field with three (3) years of experience in quality improvement in a health care or social service setting.
Additional Requirements:- Occasional overnight travel.
- Overtime and/or weekend work with Deputy approval.
- Lifting requirements: 20 lbs.
- A valid driver's license is required.
Preferred Qualifications - Proven experience in facilitating interdisciplinary team collaboration with a focus on health care settings
- Expertise in using quality improvement methodologies such as Plan-Do-Study-Act (PDSA), Lean Six Sigma, Institute for Healthcare Improvement, or other improvement frameworks
- Familiarity with evidence-informed or evidence-based maternal care models such as AIM patient safety bundles or similar practices
- Knowledge of value-based payment models
- Skilled in data collection, analysis, and performance measurement to inform continuous quality improvement decisions
- Excellent communication, facilitation, and presentation skills, with the ability to engage stakeholders at different points in the maternity care delivery system
- Experience with learning collaboratives or other collaborative initiatives
- Familiarity with electronic medical records and data management for quality improvement
- Excellent skill at managing multiple tasks and priorities in a fast-paced environment
