Beaver Dam Nursing & Rehabilitation Center is seeking a dependable and compassionate Charge Nurse (RN or LPN) to join our nursing team on a PRN basis.
The Charge Nurse provides direct nursing care while overseeing the day-to-day activities of nursing assistants and helping ensure residents receive safe, high-quality, person-centered care. This role is ideal for a nurse who is confident in clinical decision-making, enjoys leading a team, and wants the flexibility of PRN scheduling.

Key Responsibilities
- Provide direct nursing care and oversee nursing assistants during the assigned shift.
- Administer medications and treatments in accordance with physician orders and facility procedures.
- Assess residents and promptly communicate changes in condition to physicians, families, and appropriate clinical leadership.
- Coordinate admissions, transfers, discharges, and resident care activities.
- Maintain accurate and timely clinical documentation.
- Ensure resident care plans and physician orders are followed.
- Monitor medication, treatment, narcotic, and clinical documentation for accuracy and compliance.
- Provide guidance and support to CNAs while promoting teamwork and accountability.
- Respond to resident needs and assist with clinical procedures as appropriate.
- Support quality assurance, infection prevention, resident safety, and regulatory compliance.

Requirements
Requirements
- Current, unencumbered Kentucky RN or LPN license.
- Current CPR certification.
- Long-term care or skilled nursing experience preferred.
- Strong clinical assessment, documentation, communication, and leadership skills.
- Ability to supervise and support nursing assistants effectively.
- Ability to prioritize resident needs and make sound clinical decisions in a fast-paced environment.
- Ability to maintain resident confidentiality and work collaboratively with residents, families, physicians, and the interdisciplinary team.
- Ability to meet the physical requirements of the position, including prolonged standing/walking and lifting, pushing, pulling, or transferring 50 pounds or more as needed.
