Job Description
JOB OVERVIEW: The Vision Coordinator provides administrative and patient care coordination support for the ophthalmology services of a free and charitable clinic. This position serves as the primary liaison between patients, volunteer ophthalmologists, surgical facilities, and community partners to ensure patients receive timely, compassionate, and well-coordinated eye care and surgery.
GENERAL RESPONSIBILITIES
Surgical Referral & Patient Coordination
● Review patient charts in which a physician has identified a need for vision surgery.
● Track each patient from the initial surgical recommendation through referral, consultation, scheduling, surgery, and follow-up as appropriate.
● Confirm that referrals from SLO Noor Foundation have been successfully received by the appropriate surgeon or surgical office.
● Contact surgeon offices and surgical centers to verify patient information, referral status, appointment dates, surgical dates, and other necessary details.
● Refax referrals, medical records, or other documentation when necessary.
● Request medical records and ensure that required records are received and appropriately documented.
● Communicate with patients by telephone regarding their referrals, appointments, surgery dates, preparation instructions, and other questions they may have.
● Confirm patient understanding of scheduled appointments and the importance of attending.
● Reinforce with patients that participating physicians and surgical teams are generously volunteering their time to provide free surgical care and that keeping scheduled appointments is essential to respecting their time and maintaining the availability of the program for other patients.
● Identify potential barriers that may prevent a patient from attending surgery or completing the referral process and communicate concerns to the appropriate SLO Noor Foundation staff.
● Maintain accurate and timely documentation of all communications and referral activity.
Communication with Surgical Partners
● Develop and maintain effective working relationships with participating ophthalmologists, optometrists, surgeons, surgical centers, hospitals, and their staff.
● Serve as a consistent point of contact between SLO Noor Foundation and participating surgical offices.
● Verify points of contact at each participating facility and maintain current contact information.
● Follow up regularly on outstanding referrals, records, appointments, and surgical scheduling.
● Help resolve communication or documentation issues that could delay patient care.
Program Organization & Documentation
● Create and maintain program binders and other organizational tools for participating facilities and SLO Noor Foundation staff.
● Develop and maintain:
○ Vision surgery program guides
○ Referral tracking tools
○ Facility points-of-contact lists
○ Surgical workflow checklists
○ Patient coordination checklists
○ Facility-specific procedures and instructions
○ Other resources necessary for consistent program operation
● Keep program materials current and update them as procedures, contacts, or participating facilities change.
● Ensure that each participating facility has clear information about what the SLO Noor Foundation vision program provides and how referrals and surgical patients should be handled.
Vision Program Projects
● Assist with special vision-related projects as assigned by the Executive Director and Vision Director.
● Support the ongoing development, organization, and expansion of SLO Noor Foundation’s vision services.
● Assist with program improvement initiatives, data tracking, outreach, and other projects that strengthen the vision program.
● Identify opportunities to improve communication, efficiency, and patient experience within the vision program.
