Hospice of Humboldt is looking for a Case Manager Nurse to join our team. This position is 3/4-time, approximately 32 hours a week and comes with full benefits.
The Case Manager is responsible for assessing the needs of hospice patients and coordinating their care with the interdisciplinary team. The Case Manager provides direct nursing services in the patient's home or place of residence.
Minimum Requirements are as follows:
- Graduate of an accredited nursing program.
- Current California Registered Nurse license.
- Minimum of one (1) year of experience as a professional nurse within the previous three (3) years or equivalent experience in a BSN program.
-Strong clinical assessment skills.
-Excellent communication and interpersonal skills.
-Ability to work independently and as part of a team.
-Compassionate and patient-centered approach to care.
-Proficiency in electronic medical record (EMR) systems and other workplace technology.
-Valid driver's license and reliable transportation.
