About the Role:

As a Licensed Practical Nurse (LPN) at Concordia Senior Living, you will play a crucial role in providing high-quality care to our residents in a skilled nursing facility. Your primary responsibility will be to assist in the daily living activities of residents while ensuring their comfort and safety. You will collaborate with a multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each resident. This position requires a compassionate approach, as you will be building relationships with residents and their families to promote a supportive environment. Minimum Qualifications:

Current and valid LPN license.
Completion of an accredited practical nursing program.
Basic Life Support (BLS) certification.

Preferred Qualifications:

Experience working in a skilled nursing facility or long-term care setting.
Certification in geriatric nursing or related specialty.
Proficiency in electronic health record (EHR) systems.

Responsibilities:

Administer medications and treatments as prescribed by physicians.
Monitor and document residents' health status and report any changes to the nursing staff.
Assist residents with daily activities such as bathing, dressing, and eating.
Provide emotional support and companionship to residents and their families.
Participate in care plan meetings and contribute to the development of individualized care strategies.
